Description

Premiums, drug lists, and provider networks can change each year. A short review helps you spot changes before they surprise you in January, and gives you the chance to switch if a different plan fits better.

If you’re in a Medicare Advantage plan, you can make one plan change during this window.

Checklist

Bring this to your review

Gather these before we meet and bring them to our appointment. Please don’t send any of this through the website.

  • Your Medicare card and any current plan ID cards
  • Your plan’s Annual Notice of Change (ANOC) letter
  • A list of your doctors and specialists
  • A list of your prescriptions, with doses
  • Your preferred pharmacy
  • A rough monthly budget for premiums and out-of-pocket costs
  • Your questions: write them down as you think of them

Before your appointment

Before we meet to discuss Medicare plans, you’ll receive a short Scope of Appointment form to sign electronically. Medicare requires it, and it simply confirms which topics you’d like to cover.
